Visits

Labs
Basic labs to include: CBC, CMP, TSH, HA1c, Lipid Panel. Comprehensive labs to include: CBC, CMP, TSH, FT4, FT3, HA1c, Insulin, NMR Lipid Profile, Vitamin D, Uric Acid, GGT, Ferritin, hs-CRP, hormones, PSA. Specialized functional medicine testing or labwork in addition to the above will be the responsibility of the patient. It can be billed to insurance, however, coverage varies by insurance company.
Classes
Classes will be held bimonthly. One class will focus on adult related topics such as managing weight, best diets, diabetes management, sleeping soundly, solving your fatigue, brain fog, etc. The second class will focus on common parenting questions and concerns such as managing colic, helping kids eat a healthy diet, mantaining gut health in children, curbing the sugar epidemic, helping kids get off video games and outside, etc.
Educational Resources- Handouts/ Videos
Access to educational materials covering both adult and pediatric issues, such as diet tips, exercises for various musculoskeletal issues, colic tips, breathing techniques, allergy helps, best self care products, tips for better sleep, probiotic information, etc, etc. Videos covering various health topics will also be coming soon.

Enrollment Fee
Dr. Black spends a considerable amount of time reviewing each patients medical history and initial intake questionairres, along with previous tests and labwork. This covers the time he spends getting to know your history. Onboarding fee is waived for those who pay A la Carte for initial and follow up visits and then sign up for monthly membership.

Dr. Black will thoroughly review this prior to your visit. Visit will include body composition analysis (if 12 or older), time to go over your health goals, questions, symptoms, and concerns. A plan will be made for labs/further testing. A follow up visit will be made within 7-10 days to go over your personalized healing plan.
What is included in the follow up visit?
Once Dr. Black has received all lab work and testing discussed in the initial visit, he will review everything. You will then meet to go over a detailed plan to address your personalized plan to address your concerns and achieve optimal health. Subsequent follow up visits will be same as follow up price or a membership package can be purchased (onborading fee will be waived).
