Have you ever wished you could order lab work for yourself and have it interpreted by a doctor without an appointment?
It is possible through Connected Health.
Select the lab work you would like to have done and pay for testing. Rupa health will provide detailed instructions for each test including a location near you to have your blood drawn. Once testing has been returned and results are finalized, Dr. Black will provide you with a detailed interpretation of your labs.

FUNCTIONAL MEDICINE PACKAGES
1 Year Weight Loss Package ($3000 or $250/month x 12 months)
In order to maintain a healthy weight, our bodies must have the appropriate kinds of food in the appropriate amounts and be functioning optimally to process that food. You will receive an in-depth lab evaluation to look at your metabolic function to ensure your body is burning fuel like it should. You’ll be given an individualized diet plan that is doable along with a detailed exercise plan. Supplements and medications, along with lifestyle interventions specific to your bodies need will be addressed.

- Initial evaluation including lab evaluation
- 6 follow-up appointments with Dr. Black
- Personalized weight loss plan
- Access to discounted GLP medication, if needed
- 12 group classes. Monthly 1 hour classes designed to educate, teach new skills, and instill simple habits that will help you loose and keep off weight. (Classes will be recorded if you are not able to attend in person).

1 Year Diabetes Package ($3000 or $250/month x 12 months)
Want a diabetes treatment program with a goal to reverse diabetes and get you off medication. After a detailed evaluation, you will not only receive a personalized plan, but education and skills to heal and thrive. As they say, knowledge is power.

- 7 visits (In-depth initial visit plus 6 follow up visits)
- 5 sets of labs (initial, 3 month, 6 month, 9 month, 12 month)

Cardiovascular health lab evaluation and optimization ($1250 or $210/month x 6 months)
